先天性皮膚カンジダ症の1例

CONGENITAL CUTANEOUS CANDIDIASIS

　新生児期に発病する皮膚カンジダ症は少なくないと思われるが，胎児が子宮内で感染し，生下時すでに皮疹の認められるものはきわめてまれで，現在までに報告されている症例は10例に満たない。われわれは最近出生時に，ほとんど全身に皮疹の見られたカンジダ症の1例を経験したので報告し，さらにその感染機序などいくつかの問題点について考察したい。

Only 7 cases with generalized candidial eruptions at birth have been reported.

The mother of this patient had nothing particular during the pregnancy. The eruptions were composed of many miliary papulovesicular rashes on diffuse erythema, which were followed by many pustules a few days later. There were white pseudomembraneous lesions on the oral mucosa several days after birth, while candidial onychia and paronychia could not be found.

Candida albicans was cultivated from lesions of the skin and the oral cavity of the patient and from vaginal exudate of the mother.

Histologic specimen showed hyperkeratosis, subcorneal bullae, and spongiosis in some parts. Fungal elements were proved in the horny layer by PAS stain.

Topical application of an anticandidial drug for 25 days cured the lesions completely.
