　はじめに

　耳下腺腫瘍は病理組織学的に多彩な像を呈することが，その特徴の1つとされている。上皮筋上皮癌は病理組織学的に核の異型性は軽度であるが，臨床的に周囲組織への浸潤，再発，頸部リンパ節転移や遠隔転移を示す低悪性癌であり，1990年に改訂された唾液腺腫瘍のWHO組織分類に新たに加えられた^1）。しかしその報告は少なく，本邦における耳下腺原発の上皮筋上皮癌は7例を数えるにすぎない^2〜8）。筆者らは，最近耳下腺に発生し顔面神経麻痺をきたした本腫瘍の2例を経験したので，症例の臨床経過に文献的考察を加えて報告する。

Epithelial-myoepithelial carcinoma (EMC) is an infrequent and low grade malignant tumor of the salivary gland. In a 73-year-old female and a 74-year-old male, their chief complaints are a swellingof the unilateral parotid gland and facial nerve paralysis. Total parotidectomy with resection of the facial nerve involved by the tumor were underwent in each patient. Immediate facial nerve grafting using the greater auricular nerve or suspension using Gore-Tex® soft tissue patch was underwent. Histopathological examination revealed EMC with its characteristic findings of two layer arrangement of tumor cells. Their postoperative courses were uneventful.
