　はじめに

　耳下腺悪性腫瘍の組織像は多彩であり，1990年のWHO分類では18種類に細分類されている^1）。以前は明細胞腫瘍の中に包括され，核の軽度異型，臨床的に再発，浸潤傾向，遠隔転移を示す低悪性腫瘍が上皮筋上皮癌として新たに付け加えられた^2）。

　今回筆者らは，右耳下腺に発生した上皮筋上皮癌の1症例を経験したので，若干の文献的考察を加え報告する。

A 52-year-old male has a complaint of a swelling and pain of the right infra-auricular area.

Local findings and topographical imagings in-dicated this tumor was malignant parotid tumor. Subtotal parotidectomy with supraomohyoid neck dissection was performed with preservation of the facial nerve.

Skin defect was repaired with a rectus abdominal free graft flap.

Histopathological examinasion showed epitheli-al-myoepithelial carcinoma consisting of alveolar and tubular formations. and immunohistochemical examinasions showed positive S-100 protein.
