　はじめに

　鼻・副鼻腔乳頭腫症例に対する外科的アプローチとしては，Caldwell-Luc法やDenker法が用いられていたが，再発が高率にみられることによりlateral rhinotomy （外鼻切開）法またはmidfacial degloving法による切除術が推奨されるようになった^1）。また，内視鏡下手術の進歩により，一部の症例に対しては内視鏡下手術の適応もある^2）。

　Midfacial degloving法は，1974年にCassonら^3）が報告した方法で，歯肉口腔前庭粘膜切開と鼻中隔コルメラを貫通する切開，外側鼻軟骨と大鼻翼軟骨の間での剥離により，顔面皮膚を両側で挙上する術式である。つまり顔面に皮膚切開を行わなくても，顔面皮膚を広く剥離・挙上できる点が長所として挙げられている。

The midfacial degloving procedure affords excel-lent exposure and surgical access to the nasal cavityand paranasal sinuses without facial incisions. This approach has been used for the removal of sinonasal papillomas. We reported our experience with a modification of the midfacial degloving approach, in which elevation of a skin flap from the anterior wall of the maxillary sinus was avoided in the healthy side. Good exposure of the anterior wall of the maxillary sinus was obtained in this modifica-tion. Infra orbital numbness in the healthy side could be avoided.
