 First bite syndrome(FBS)is a complication of parapharyngeal space surgery that presents as pain in the parotid region and jaw, which occurs with the first bite of a meal. Sympathetic denervation of the parotid gland with subsequent hypersensitivity of myoepithelial cells to parasympathetic stimulation is suspected as pathogenesis of FBS. We report a case of FBS after parotidectomy of a deep lobe tumor, in which a botulinum toxin treatment was effective for resolution of the pain. No adverse event was observed. A botulinum toxin injection is a safe and less-invasive treatment for patients with FBS.
