はじめに

　口腔底，オトガイ部，顎下部に発生する囊胞性疾患として類皮囊胞，ガマ腫，囊胞状リンパ管腫，正中頸囊胞などが挙げられる。術前診断としてCT，MRI，頸部超音波検査，細胞診などが有用であるが，診断に難渋し術前診断と術後診断が一致しないことも稀ではない。われわれは過去15年間でオトガイ部に発生した腫瘤の手術を13例経験した。そのうち口外法（頸部外切開）により摘出した巨大な類皮囊胞例を呈示し，そのほか12例を含め文献的考察を加え報告する。

　We presented 13 cases of a dermoid cyst of the submental region and considered classification of dermoid cysts and surgical approach. Dermoid cysts are classified by the physical relationship with the mylohyoid, as type of the floor of mouth and submental type. The one with a diameter more than 6 cm are generally referred as “Giant dermoid cyst”. The first choice of treatment is surgical operation by intraoral approach or neck incision approach. Surgical approach should be considered by its positon, size, and preoperative differential diagnosis of dermoid cyst.
