鼻腔腫瘍と誤診された鼻腔内encephalomeningoceleの1例

A Case of Intranasal Encephalomeningocele Diagnosed as a Nasal Neoplasm

I．はじめに

　encephalomeningoceleは頭蓋骨の裂隙から脳が脱出する先天的な奇形で，後頭部に発生することが多い。耳鼻咽喉科領域に出現することは稀ではあるが，鼻茸と間違えて手術し髄膜炎などの危険な状態を招くことがあるので留意する必要がある。われわれは初め鼻腔腫瘍を疑って摘除を試み，手術の途中でencephalomeningoceleと気づき，あらためて根本的な手術を行った症例を経験したので報告する。

A 3-year-old girl was referred to our clinic because of difficulty in breathing through the nostril.

The left nasal cavity was completely filled by a greyish, polypoid tumor, which pressed the nasal septum strongly toward the right. Aspiration performed during the operation revealed the tumor to be encephalomeningocele. Six months later an intracranial resection of the encephalomeningocele was performed without complication.

In the present paper the significance of the differential diagnosis of encephalomeningocele and nasal polyp (or nasal neoplasm) is discussed.
