下顎骨骨髄炎が原因と考えられた深頸部ガス壊疽の1例

Osteomyelitis of the mandible resulting in gas gangrene of the neck in a hemodialysis patient

はじめに

　ガス壊疽とは，軟部組織壊死を引き起こす軟部組織内感染症のうち，嫌気性菌感染によって体内にガス産生を生じた壊疽性感染症の総称で，嫌気性桿菌であるClostridium属を起炎菌とするClostridium性と非Clostridium性に大別される。その臨床症状は急速に進行し，局所所見では組織内ガス像や筋組織破壊に伴う激しい疼痛，著明な発赤腫脹などを認め，敗血症やdisseminated intravascular coagulation（DIC）などの重篤な病態に陥る危険性を有している。深頸部感染症は歯原性の炎症や，扁桃炎や顎下腺炎といった耳鼻咽喉科領域の感染症の続発症として発症することが多く，形成された膿瘍や組織内ガスは頭頸部領域の疎な頸部組織の間隙を通じて縦隔にまで進展することもあり，縦隔炎を生じた場合には死亡率が上昇するといわれている^1〜7）。ガス壊疽の治療には，切開・排膿・デブリドマンといった外科的処置，感染部位の生理食塩水洗浄，起炎菌に対して感受性を有する抗菌薬の投与が必要であり，特に抗菌薬投与は長期間継続して必要となる場合もある。今回われわれは，下顎骨骨髄炎が原因と考えられた右深頸部ガス壊疽を発症した血液透析症例を経験したので，その概要と経過に文献的考察を加えて報告する。

　A 74-year-old man undergoing chronic hemodialysis treatment developed an infection of the deep portion of the right neck with secondary gas production that extended outside of the mandibular ramus and into the parapharyngeal space. We performed emergency neck drainage surgery and antibiotic therapy. Culture of the abscess contents showed anaerobic Streptococcus constellatus and Prevotella buccae. Subsequent imaging revealed osteomyelitis of the mandible with cortical bone destruction as the probable cause of the deep neck abscess. Because the patients undergoing chronic hemodialysis treatment are relatively immunocompromised, a multidisciplinary approach is often required for such patients with this type of infection.
