深頸部ガス形成性膿瘍の1例

Deep Neck Infection:Report of a Case

　はじめに

　抗生剤の出現以前には，深頸部膿瘍は，重篤でしばしば致死的な疾患であった。抗生剤の進歩によって，その頻度は少なくなってきている。一方，抗生剤による症状の隠蔽化や耐性菌の関与によって病態は複雑になっている。保存的治療で軽快するものもあるが，頸部外切開，ドレナージの必要性は従来同様に重要である。

　私達は，深頸部膿瘍に伴い頸部にガスを生じ，治療によって救命し得た症例を経験したので，若干の文献的考察を加えて報告する。

Deep neck infection, which affect soft tissues and fascial compartment of the head and neck, can lead to lethal complications. Recently, owing to the advance of antibiotics, the incidence of these serious complications has decreased, but the diagnosis and the subsequent management was more difficult because of masking effect on the old clinical signs.

A 63-year-old female patient was referred to our clinic with deep neck infection. She was treated with surgical incision and drainage, and chemotherapy. Cultures from the neck abscess grewPeptostreptococcus and Bacteroides (anaerobes cul-ture). The therapy was effective and the patient made successful and complete recovery.
