I．はじめに
破傷風（tetanus）は，破傷風菌（Clostridium tetani）によって産生される毒素の1つである神経毒素により，強直性痙攣を引き起こす感染症である。1968年より乳児への破傷風トキソイドの接種が開始され，その罹患数や死亡者数は激減しているものの，いまだに年間100人程度の発症がある1）。重篤な場合には死に至ることもあるため，破傷風はいまだに重要な疾患である。不幸な結果を招かないためには，早期の診断（疑いも含めて）とICUでの全身管理を含めた適切な治療が重要である2）。全身の強直性痙攣の前駆症状である開口障害，嚥下障害を呈した患者2）が耳鼻咽喉科を受診する可能性は高く，耳鼻咽喉科医にとって常に念頭に置かなければならない疾患である。今回，われわれは急速に進行した破傷風で，早期診断が救命につながったと思われる1症例を経験したので報告する。
Tetanus is a severe infectious disease because some patients die with tonic convulsion. Recently, in spite of a wide use of vaccination, there is an outbreak of about 100 tetanus patients per year in Japan. Because the initial symptoms, which include trismus and inability of swallowing, are occurred in oral area, tetanus patients often consult with otorhinolaryngologists. For avoiding the death, the early diagnosis and the appropriate therapy are important. We report a case of a tetanus patient whose symptoms progressed rapidly. We recognized the importance of tetanus for the differential diagnosis of trismus and inability of swallowing.
