ICT（infection control team）と医療情報部の立場からみた耳鼻咽喉科病棟の多剤耐性菌の現状―2001年度1年間の実践より

The Status Quo of the Drug-Resistant Bacteria in ENT Ward in Terms of the ICT（Infection Control Team） and the Medical Information System

I．はじめに

　近年，病院のリスクマネージメントの1つとして，また対費用効果の点から院内感染対策は重要な問題となっている。各病院においてチームを編成し，多剤耐性菌をはじめとする病原体の検出状況の分析，院内感染予防の対策がなされ，その結果について情報開示がなされている。マニュアルも整備され，他院への参考のために公開もされている^1,2）。しかし，依然として院内感染の発生が散発的に報道されている。また，いったん院内感染が発生すると，莫大な時間的，経済的損害および心理的な障害が患者のみならず病院スタッフにも生じる。一方，メチシリン耐性黄色ブドウ球菌（MRSA）は医療従事者の鼻腔^3），さらには高齢者では咽頭からも検出され^4），耳漏や喀痰からもしばしば検出される。また，通常は気道には常在化しないとされる緑膿菌は，無症候性の咽頭からも検出される^5）。ところが，耳鼻咽喉科病棟は混合病棟であることがほとんどで，他科患者からの感染が危惧される。

　今回われわれは，当院全体（以下，全体と略）と耳鼻咽喉科病棟（以下，病棟と略）における多剤耐性菌の検出の状況を報告するとともに，院内感染予防対策と耳鼻咽喉科特有の問題点について検討を加える。

　The investigation was performed regarding the prevalence of drug resistant bacteria mainly focusing on Methicillin resistant staphylococcus aureus（MRSA）,pseudomonas aeruginosa（P. aeruginosa）,and serratia marcescens in ENT ward from April,2001 to March,2002 by the infection control team. No nosocomial infection caused by these bacteria was confirmed,although the colonization was found in sputa and/or pus in patients with malignancy or viral infection. These results indicate that a universal precaution is of a great importance to prevent the hospital acquired infection,especially where a large number of the patients belonging to the other departments are admitted with immuno-suppressive patients.
