皮膚瘻孔を生じた亜急性甲状腺炎（de Quervain）の1例

A Case of Subacute（de Quervain's） Thyroiditis Accompanied with a Fistula of Skin

I．はじめに

　亜急性甲状腺炎は，1894年にMygindによりthyroiditis acuta simplexとして初めて報告され^1,2），その後1904年にde Quervainが文献より67例を集め，その特異的な病理像を急性非化膿性甲状腺炎として報告したものである^2～4）。

　熱発，甲状腺の疼痛，全身症状をもって急激に発症し，放置していても数か月で自然治癒することを特徴とする疾患とされる。

　しかし，今回われわれは，無痛性の前頸部腫脹として発症し，自然治癒せず経過中に皮膚瘻孔を形成，病理学的に亜急性甲状腺炎と診断された非典型的な症例を経験したので，その発症機転について文献的考察を加え報告する。

　A 60-year-old man presented with subacute（de Quervain's） thyroiditis accompanied with a fistula of skin. On his first visit,he had anterior neck swelling without pain for ten days.

　Serological test revealed elevation of serum free T₄ and thyroglobulin. The result of fine needle aspiration biopsy of the mass was class II.

　Subacute thyroiditis was suspected and he was followed up without therapy. After one month,he developed an external fistula of skin of the anterior neck.

　CT scan of the neck revealed abscess in the left lobe of the thyroid. The abscess was not responsive to antibiotic therapy. The patient underwent lobectomy of the thyroid gland. During the operation,no duct of the piriform sinus fistula was identified.

　Pathologic exanimtion of the left lobe of the thyroid gland revealed subacute（de Quervain's） thyroiditis. No recurrence was observed after the surgery.
