I．はじめに

　上顎に囊胞を形成し，頬部，鼻前庭，口蓋などに腫脹をきたす疾患は多数存在する。これらの中に胎生期に存在し生後1年以内に消退するとされる鼻口蓋管（切歯管）の遺残上皮により形成され非歯原性囊胞である切歯管囊胞は，比較的稀なものである^1～3）。

　今回われわれは，31歳の男性で硬口蓋および鼻前庭に著明な膨隆をきたし，巨大な切歯管囊胞と考えられた1症例を経験し，内視鏡下鼻内手術により囊胞を開放したので文献的考察を加えて報告する。

　Nasopalatine duct cyst is a comparatively rare disease. A 31-year-old male presented with a nasopalatine duct cyst of 60×40×30mm in size defect in the middle portion of the hard palate. The patient complained of a palatal swelling and nasal obstruction. CT scan showed a giant cystic mass in his upper jaw which produced of deformity of his nasal and paranasal structure. Based on WHO classification,nasopalatine duct cyst was diagnosed. Using endoscopy,a wide opening was made by mucosal resection in the floor of the nasal cavity over the cyst roof. As the endonasal surgery is easy for ENT surgeons,we recommend endonasal surgery to treat the nasopalatine duct cyst.
