I．はじめに

　鼻性頭蓋内合併症は，副鼻腔に感染源があり，それにより頭蓋内に感染をきたすものである。抗菌薬の進歩した現在では発生頻度は低下してきているものの，いったん発症すれば重篤な経過をたどることがあり，死亡率は10％前後とされている^1～6）。また，感染源である副鼻腔炎の処置が不十分なまま頭蓋内病変が遷延すると，片麻痺や失語症などの神経学的な後遺症を残すこともある^7）。このため早期に診断し，原発巣に対する迅速で適切な治療を行い，その後も頭蓋内病変に対する厳重な経過観察が必要となる。

　今回われわれは鼻性頭蓋内合併症の2症例を経験し，後遺症を残すことなく治癒させることができたので，その経過と治療法の概要を述べ，若干の文献的考察を加えて報告する。

Rhinogenic intracranial complications are still serous,although the development of antibiotic therapy has reduced its incidence. We report two cases of rhinogenic intracranial complications. The first case was a 29-year-old male who had meningitis and subdural abscess secondary to acute ethmoidal sinusitis. We performed endoscopic sinus surgery（ESS）immediately. The second case was a 32-year-old male who had subdural abscess secondary to acute frontal sinusitis. We performed transfrontal approach surgery and ESS immediately. In case of rhinogenic intracranial complications,prompt diagnosis and early surgical treatments are important.
