結膜下悪性リンパ腫に続発した開放隅角緑内障の1例

A case of secondary open-angle glaucoma associated with subconjunctival malignant lymphoma

（1）両側性の結膜下腫瘍を伴った開放隅角緑内障の1例を経験した．

（2）この結膜下腫瘍は組織学的には悪性リンパ腫（びまん性リンパ腫小細胞型）であった．

（3）結膜下腫瘍を切除したところ，眼圧は下降し，C値にも改善が見られたことより，結膜下悪性リンパ腫が直接上強膜静脈を圧迫し，房水流出抵抗が増大し，緑内障を起したものと考えた．

A 69-year-old female manifested increasedintraocular pressure with moderate glaucomatous visual field defects in both eyes. She was also suffering from bilateral hyperemia and lacrimat ion for the past 3 years. We diagnosed her as subconjunctival malignant lymphoma with secondary glaucoma bilaterally.

The lymphoma was located along the corneal limbus extending over 270° in the right eye and 210° in the left. Surgical resection of the lymphoma was promptly followed by normalization of the intraocular pressure and improvement in the facility of outflow. There has been no relapse nor signs of metastasis of the lymphoma during the postoperative follow-up period of 16 months. The presence of subconjunctival lymphoma appears to have been the major cause of glaucoma in this patient.
