　二つの異なった方法で行った白内障手術と眼内レンズ挿入術876眼についての結果を検索した。ブリッジ切開と無縫合自己閉鎖方式の176眼（A群），ポケット切開と4X連続縫合の600眼（B群），無縫合自己閉鎖方式と角膜減張切開術を併用した100眼（C群）である。術前乱視は，A群では1D以内，C群では1D以上平均1.8Dであった。術翌日の乱視は，A群では平均2.9Dであり，3か月後には平均0.3になった。B群では，術翌日の平均が0.2Dであり，術後1年間この値を保っていた。C群では，術翌日の平均が0.3Dで，2か月後には平均0.2Dであった。術後1か月での0.5以上の裸眼視力は，A群では42％，B群では69％，C群では83％で得られた。以上の事実から，術前倒乱視が1D以上の白内障眼に対しては，無縫合自己閉鎖方式と角膜減張術を併用することが有用であると結論される。

I reviewed the result of cataract surgery with intraocular implantation in three groups. In 176 eyes (group A), I employed bridge incision with self -sealing no-stitch wound closure. In 600 eyes (group B), I employed standard pocket incision and continuous 4-X suture. These eyes showed astigma-tism of less than 1 diopter (D) prior to surgery. In 100 eyes (group C), I used self-sealing no-stitch wound closure with simultaneous astigmatic ker-atotomy. These eyes showed astigmatism of more than 1D, average 1.8D, prior to surgery. Astigmatic keratotomy was performed using a nomogram modified from that by Thornton.

Postsurgical astigmatism in group A averaged 2.9D one day after surgery. It decreased to 0.3D 3 months after surgery. Eyes in group B showed astigmatism of 0.2D one day after surgery. The value stayed stable at least for one year after surgery. Eyes in group C showed astigmatism aver-aging 0.3D the day after surgery. It averaged 0.2D 2 months after surgery. At one month after surgery, uncorrected visual acuity of 20/40 or better was obtained in 42% of eyes in group A, in 69% in group B and in 83% in group C. No-stitch cataract sur-gery with astigmatic keratotomy is recommended for eyes with presurgical astigmatism of 1D or more.
