Abstract 文献概要

　原因不明の視力低下がある25歳男性とその20歳の妹を検索した。兄の矯正視力は右0.1，左0.15，妹のそれは左右とも0.3であった。眼底と蛍光眼底造影には異常所見がほとんどなかった。網膜電図（ERG）では，両名とも杆体系と錐体系とに振幅低下と頂点潜時の延長があった。兄のERGは，杆体系，錐体系ともnegative typeであった。妹では錐体系ERGの刺激強度を高めたときにのみnegative　typeを呈し，黄斑部局所ERGは消失型であった。これらERG所見から，両名とも病期が異なる杆体—錐体ジストロフィと診断した。ERGがnegative typeを示したことから，on型双極細胞の障害が併発していると推定した。

A 25-year-old male and his 20-year-old sister presented with decreased visual acuity. The corrected visualacuity of the brother was 0.1 right and 0.15 left. That of sister was 0.3 in either eye. Funduscopy and fluorescein angiography showed almost normal findings. Electroretinogram (ERG) showed reduced amplitudes for rod and cone systems withincreased peak latency. ERG in the brother showed negative type for rod and cone systems. ERG in the sister showednegative type response for higher intensity with extinguished focal macular response. These ERG findings led to thediagnosis of rod-cone dystrophy in different stages in the siblings. The negative-type ERG response seemed to suggestthe additional presence of dysfunction of bipolar cells.
