視神経乳頭サルコイドーシスと副腎皮質ステロイド薬の全身大量療法

Systemic megadose corticosteroid therapy for sarcoidosis with optic disc granuloma

　21歳の男性が1週間前からの左眼の視野欠損で受診した。矯正視力は右1.5，左1.0であった。両眼に前房内細胞，隅角結節，雪玉状の硝子体混濁があった。左眼眼底に，4×5乳頭径で高さが約8Dの視神経乳頭肉芽腫とこれに接する漿液性網膜剥離があった。全身検査と頸部リンパ節生検でサルコイドーシスの診断が確定した。初診から4週後にコハク酸メチルプレドニゾロンナトリウム（ソル・メドロール^TM）の200mg／日点滴静注を開始し，その後メチルプレドニゾロン24mg／日の内服に切り替えた。治療開始後2か月で肉芽腫はほぼ消退した。視神経乳頭肉芽腫例があるサルコイドーシスに，副腎皮質ステロイド薬の大量療法が有効であることを示す症例である。

A 21-year-old male presented with visual field defect in his left eye since one week before. His corrected visual acuity was 1.5 right and 1.0 left. Both eyes showed cells in the aqueous, nodules in the chamber angle and vitreous opacity simulating string of pearls. Funduscopy of the left eye showed a large granuloma at the optic disc surrounded by serous retinal detachment. Systemic examinations including lymph node biopsy in the neck led to the diagnosis of sarcoidosis. He was then treated by intravenous methylprednisolone at the daily dosis of 200 mg and later by oral methylprednisolone 24 mg daily. The optic disc granuloma showed a complete remission after 2 months of treatment. This case illustrates the usefulness of systemic megadose corticosteroid therapy for sarcoidosis with optic disc granuloma.
