A 58-year-old female presented with itchiness in both eyes. She had been diagnosed with schizophrenia at the age of 20. She had been treated by systemic phenothiazine, bromperidol, carbamazepine and metixene hydrochloride for the past 10 years. She had bilateral mild blpharitis. Her corrected visual acuity was 1.0 in either eye. She showed, in both eyes, diffuse yellowish pigmentation on the posterior surface of the cornea and similarly pigmented granules in the lens. No corneal edema was present. The density of corneal endotheleal cells was within normal range. This case illustrates that long-term ingestion of antipsychotic drugs may result in corneal and lens pigmentation without visual disturbance.
