（P1-2-50）　59歳，男性が右眼打撲による視力障害で受診した。初診時，右眼は光覚なく，眼圧は28 mmHg，眼球突出がみられた。フルオレセイン，インドシアニングリーン両螢光造影では，網膜中心動脈および外側後毛様動脈に血流障害がみられたが，内側後毛様動脈の血流は保たれていた。CT,MRI所見により，下直筋内血腫の圧迫による眼動脈閉塞症と思われた。アセタゾラミド，高浸透圧剤投与により数時間後に視力は光覚弁となった。3か月後には下直筋内血腫は消退傾向を示し，網膜中心動脈および外側後毛様動脈の血流障害は軽快していた。

A 59-year-old male presented with visual impairment in his right eye two days after blunt ocular injury. The visual acuity was no light perception in the right eye. The right eye showed exophthalmos with the intraocular pressure of 28 mmHg. Fluorescein and indocyanine green angiography showed impaired flow in the central retinal artery and temporal posterior ciliary artery. The nasal posterior ciliary artery appeared intact. Computed tomography and magnetic resonance imaging showed a hematoma within the inferior rectus muscle as the presumed cause of ophthalmic artery occlusion. The visual acuity improved to light perception a few hous after systemic hyperosmotic therapy and acetazolamide. Three months later, the hematoma became smaller associated with recovery of circulation in the central retinal artery and temporal posterior ciliary artery.
