　57歳女性の頸動脈海綿静脈洞瘻の症例に対し，超音波カラードップラー装置を用い，臨床像および理学的所見を定量的に評価した。症例は内頸および外頸動脈と海綿静脈洞とのdural shuntを有し，導出静脈は主に左上眼静脈で拍動性に富む動脈性のドップラー波形を描き，血管の高度な拡張を伴う正常と逆の赤色系の血流動態として描出された。右上眼静脈は軽度に拡張した静脈系の血流動態を示した。海綿静脈洞と内頸動脈とのshuntの塞栓術後，左上眼静脈の拍動性は減弱した。本法は本症の局所血流動態を非侵襲的に直接動的に経過観察できる有用な方法で，そのドップラー・スペクトラムは病勢と相関し，病像および治療のすぐれた指標となりうる。

A 57-year-old female developed left carotid cavernous sinus fistula, CCF. Ultrasound color doppler sonography, UCD, showed dural shunts between both internal-external carotid arteries and the cavernous sinus. The left superior ophthalmic vein served as the main drainage route, showing pulsating arterial doppler pattern and a red hue with marked dilatation. The right superior ophthalmic vein was slightly dilated showing a venous flow pattern with retrograde blood flow. The pulsation in the left superior ophthalmic vein decreased after embolization of the dural shunt 16 months after onset. This case illustrates the clinical usefulness of ultrasound doppler sonography in the diagnosis and follow-up of CCF.
