　15年前から治療を受けていた63歳女子の慢性涙嚢炎が急性悪化し，膿からメチシリン耐性ブドウ球菌が分離された。1日1回のミノサイクリンとバンコマイシンの点滴静注を開始した。バンコマイシンの血中濃度を，点滴前，点滴終了2時間後，次回の点滴前に定量し，その値を基準にして投与量を決定するとともに，腎障害を予防することができた。急性炎症は10日後に寛解した。

A 63-year-old female had been suffering from chronic dacryocystitis for the past 15 years. Methicillin-resistant Staphylococcus aureus (MRSA) was isolated during a recent acute exacerbation. She was treated daily by intravenous infusion of minocycline and vancomycin. The concentration of vancomycin in the circulating blood was monitored before infusion, 2 hours after end of infusion and just before the next infusion. This procedure enabled determination of minimum effective dosis avoiding renal impairment. Acute inflammation subsided after 10 days of treatment.
