先天上斜筋麻痺に伴う斜頸と顔面の非対称

Head tilting and facial asymmetry related to congenital superior oblique palsy

　先天上斜筋麻痺患者20名において，顔面写真を用いて顔面の非対称（facial asymmetry）の頻度と程度，およびhead tiltの方向との関係を検討した。20名中9名に有意な顔面の非対称がみられた。手術後2年以上経過を観察しえた症例は12名で，術前に有意な顔面の非対称がみられた5名のうち，術後head tiltが著明に改善した3名では顔面の非対称も正常範囲にまで改善した。手術による頭位改善により，顔面の非対称も改善されていくことが示された。

We evaluated 20 patients of congenital superior oblique palsy regarding head tilting and facial asymmetry. The condition was unilateral in all the cases. Photographic analysis showed significant facial asymmetry in 9 patients. Twelve patients were followed up for two years or more after surgery. Facial asymmetry was initially present in 5 patients and improved in 3 after surgery, presumably due to improve-ment in head tilting.
