Abstract 文献概要

　2004年2月までの18か月間に44例44眼を感染性角膜潰瘍と診断した。潰瘍部の擦過物を培養し，21例に細菌が検出された。うち6例がメチシリン耐性黄色ブドウ球菌（MRSA），1例がメチシリン耐性表皮ブドウ球菌（MRSE）であった。いずれも高齢，骨髄移植後，水疱性角膜症などの易感染性患者に生じていた。ニューキノロンとクロラムフェニコールの頻回点眼を主とする治療で，発症から3か月以内に瘢痕治癒しバンコマイシンを必要としなかった。メチシリン耐性黄色ブドウ球菌による角膜感染症の治療では，すでにバンコマイシン耐性黄色ブドウ球菌（VRSA）の病院内での頻度が増加していることもあり，バンコマイシンの使用は慎重に行うべきである。

　We diagnosed 44 eyes of 44 patients as infectious corneal ulcer during 18 months through February 2004. Bacterial organisms were isolated from scrapings of the ulcer in 21 cases. Methicillin-resistant Staphylococcus aureus（MRSA）was present in 6 cases. Methicillin-resistant Staphylococcus epidermidis（MRSE）was present in 1 case. All the 7 cases were compromised hosts including advanced age,bullous keratopathy,or history of bone marrow transplantation. All the cases were cured within 3 months following treatment including frequent intillation of newquinolon and chloramphenicol. Use of vancomycin needs utmost care because of increasing incidence of vancomycin-resistant Staphylococcus aureus（VRSA）in hospitals in Japan.
