　最近の眼圧測定は測定部分の大きさが直径7mm（圧平部分の直径3.06mm）のゴールドマン型圧平眼圧計（GAT）が標準的に使われているが，この大きさのプローブは，瞼裂幅の小さな患者で測定誤差の原因となることがわかっている。この問題を解決する方法の1つとして筆者らは圧平部分（プランジャー）の直径が1.8mm，測定先端部（ノーズピース）の直径が5.0mmと小型化された新しいMackay-Marg型眼圧計（ProTon^®）を用いて，測定を行い，また，ProTonとGATによる測定の誤差と誤差に関与する因子について調べた。対象は無差別に選択された87名の患者の87眼であり，ProTonとGATによる測定値に対してSASプログラムによる多変量解析を行った。ProTonによる眼圧測定値はGATによる眼圧測定値と有意差がなく，測定誤差は患者の年齢，矯正視力，角膜曲率半径のいずれとも相関しない（estimated difference 0.58，-0.070，-0.059）。しかし，検者の習熟の程度は測定誤差の大きさと相関し，1人の検者が10回以上の測定を経験することによって充分な水準（p＜0.05）に達すると考えられた。

　The large size of the applanation head of the Goldmann applanation tonometer may cause erroneous reading of the intraocular pressure in eyes with a narrow lid fissure. This problem may be eliminated by the use of different instruments with a smaller applanation area. We studied the intraocular pressure in one eye of 87 outpatients with the new Mackay-Marg tonometer (ProTon^®) and investigated the parameters for an erroneous measurement of the intraocular pressure.

　Parameters such as an age of the patients, corrected visual acuity, and corneal curvature did not correlate positively with the difference between the measurements by Goldmann applanation tonometer and ProTon. One significant factor was the experience of the examiner. When the repetition of an examination exceeded 10 times, the difference between the measurements by Goldmann applanation tonometer and ProTon became non-significant.
