　原因不明の片眼の充血・眼球突出・網膜静脈閉塞様出血・脈絡膜剥離が，きわめて徐々に出現した症例を経験した．検査の結果，特発性頸動脈・海綿静脈洞瘻（以下CCF）が発見され，眼症状がこれによるものと判明した．これらの症状はシャント血管の塞栓術により急速に改善した．

　特発性CCFは外傷性CCFに比べて稀で，シャント血液量が少ないため，症状が軽く，経過も慢性で診断がつけにくく，見逃されやすい．また，脈絡膜剥離は特発性CCFにはまれな合併症であり，本邦ではあまり知られていない．脈絡膜剥離の原因のひとつにCCFを念頭におくべきことを示唆する貴重な症例と考える．

A 52-year-old female gradually developed conjunctival injection, hemorrhagic retinopathy, exophthalmos and choroidal detachment in the right eye over a period of 30 months. Computed tomography (CT) and magnetic resonance imaging (MRI) showed enlarged superior ophthalmic vein. Carotid angiogra-phy showed arteriovenous malformation in the cavernous sinus and led to the diagnosis of carotid cavernous fistula. The clinical signs rapidly resolved following transarterial embolization of the fistula. This case illustrates that choroidal detachment may result from carotid cavernous fistula.
