キノロン系抗菌剤DR−3355のヒト涙液内移行

Penetration of l-ofloxacin (DR-3355) into the lacrimal fluid

　キノロン系抗菌剤DR−3355のヒト涙液内移行濃度を測定し，薬動力学的に解析した。同時に涙液採取法の違いによる測定濃度の差について検討した。DR−3355はofloxacinの光学異性体（l体）である。この薬剤を正常男性健康人13名（24〜39歳，平均年齢32歳）に，空腹時1錠（100mg）を経口投与したうえで，投与後0.5，1，2，4，8，12時間時点で涙液を採取した。涙液採取は1回の薬剤投与に対し1時点のみとした。涙液採取はガラス毛細管を用いて，涙液メニスカスから直接採取する方法（第一法）と，あらかじめ下結膜嚢に生理食塩水を20μl点眼したうえで，涙液を回収する方法（第二法）を用いた。両方法とも薬剤投与後0.5時間より涙液中への移行を認め，ピーク値は2時間時点で，0.61μg／ml （第一法），0.92μg／ml （第二法）であった。その後，8時間（第一法），12時間（第二法）まで涙液中へのDR−3355の移行濃度が測定可能であった。涙液の採取方法による測定濃度差は，各時点で（第二法）が（第一法）を0.2μg／ml程度上回ったが，両方法の測定値の相関係数は0.9965と良好であった。

We quantitated the tear fluid level of l-oflox-acin, a new quinolone antimicrobial agent, after peroral administration of 100 mg in 13 healthy volunteers aged from 24 to 39 years, average 32 years. When tear fluid was collected from the meniscus of the precorneal film, the peak concen-tration after 2 hours averaged 0.61±0.25 μg/ml. When tear fluid was collected from the cul-de-sacafter instillation of 20μl of physiological saline, the peak concentration after 2 hours averaged 0.92± 0.67 μl/ml. Values by the two methods were well correlated. Drug levels could be detected for up to 8 hours by the first method and up to 12 hours by the second. The concentration of l-ofloxacin in the tear fluid was similar to other quinolone antimi-crobial agents.
