　Continuous circular capsulorhexis （C．C．C．）を用いて完全嚢内固定とされていた後房レンズ（PCL）が，術後次第に偏位をきたした2症例を経験した。より大きく残った前嚢側へ偏位しており，従来の一般的な偏位の方向とは反対側であった。

　その機序として，前後嚢の癒着に伴って光学部縁の一部がまず露出することが重要であり，前嚢縁—光学部—後嚢の3者が接することによりレンズが偏位していくものと推測された。

　この偏位を予防するためには，術後長期にわたって光学部が前嚢に覆われていることが重要で，C．C．C．の習熟，C．C．C．直径の縮小，large opticsレンズやPMMA支持部レンズの採用が有効であると示唆された。

We observed 2 cases of decentration of completein-the-bag fixated intraocular lens implanted by continuous circular capsulorhexis technique. In both instances, the implanted lenses were decenter-ed toward the wider anterior capsule remnant. A partial baring of the lens optics seemed to be a major factor for the process of decentration. We advocate that the edge of the anterior capsule, the lens optics and the posterior capsule be kept apart to prevent eventual decentration.
