Abstract 文献概要

　Endocapsular techniqueによる前嚢切開後，後房レンズを挿入した190症例，248眼について，嚢内固定率および固定位置別の術後合併症の種類・頻度の比較を行い，endocapsular tech—niqueの問題点を検討した。

　1．固定率は嚢内固定（In-In）79.0％，非対称性挿入（In-Out）10.9％，？外固定（Out-Out）2.0％，計画的嚢外固定（planned Out-Out）2.0％，不明6.2％であった。

　2．術後視力，角膜乱視度，乱視軸，術前屈折予測値と術後屈折実測値との誤差，術後炎症所見には，固定位置による有意差は認められなかったが，レンズの偏位，傾きはIn-Out群で有意な増加をみた。

　3．術中に後嚢破損，硝子体脱出を生じた5眼は，前嚢を残し意図的にOut-Outに挿入した。術後の視力，固定状況は良好である。

　4．In-Out群で嚢外固定と認めたループは，77％が先に挿入したループであった。

　以上より本法の問題点として，核娩出時の前嚢亀裂，非対称性前嚢切除が後房レンズの術後位置異常の主因と考えられ，今後理想的な前嚢切開としては，対称性で亀裂の生じ難い形状が必要と思われた。

We evaluated the state of fixation of posteriorchamber lens in 248 eyes, 190 patients, after sur-gery with the endocapsular technique. Whenevaluated 3 months or more after surgery, theintraocular lens was fixated in the bag in 79% ofthe eyes. The state of lens fixation was not correlat-ed with the postoperative visual acuity or the inci-dence of complications. Decentration of the lenswas noted in 33.3% of eyes with in-out fixation and2.0% of eyes with in-in fixation. In eyes with in-outfixation, the loops at 9 o'clock, which was routinelyinserted first, were located out of the bag at ahigher rate than loops at 3 o'clock. The findingsindicate that a symmetric anterior capsulotomy ismore important than the endocapsular technique toensure in-the-bag implantation.
