We evaluated the outcome of visual acuity in 92eyes with rhegmatogenous retinal detachmenttreated by pars plana vitrectomy and fluid-gasexchange. Eventual retinal reattachment wasattained in 76 eyes after a single surgery, in 14 eyesafter 2 surgeries and in 2 eyes after 3 surgeries.

Postsurgical visual acuity improved from thepresurgical level in 59 eyes (64%), remained un-changed in 11 (12%) and deteriorated in 22 (24%).The postsurgical visual acuity was not related withthe frequency of surgical intervention to induceretinal reattachment.

There were 38 eyes without macular hole withresultant retinal reattachment after a single sur-gery. Retinal detachment had involved the maculain 25 eyes and spared the macula in 13. Postope-rative improvement in visual acuity resulted in 19out of the 25 eyes (76%) and in 6 out of the 13 eyes(46%). Most of eyes with presurgical visual acuityof 20/40 or better resulted in deterioration of visionin spite of reattachment.

There were 21 eyes with macular hole with resul-tant retinal reattachment after a single surgery.Visual acuity improved in 12 eyes (57%) and de-creased in 4 (19%). Improvement in visual acuitywas more frequent when retinal detachmentinvolved 2 fundus quadrants or more than whendetachment was localized in the macular area.
