後房眼内レンズ挿入術眼における交感性反応の定量的観察

Quantitation of consensual reaction following posterior chamber lens implantation

　内眼手術を行うと，術眼では血液房水関門が破綻して術後炎症が生じるが，それが他眼に交感性に伝達し，他眼にも炎症，いわゆる交感性反応が発生すると言われている。この，交感性反応を究明するために，水晶体嚢外手術後に後房眼内レンズ挿入を行った20例で，レーザーフレアセルメーターを用いて，術眼と他眼の前房内蛋白濃度と細胞数の経時変化を測定した。術後，手術による血液房水関門の破綻を反映して，術限のフォトンカウント値と細胞数は著明に上昇したが，術後経過とともに漸次減少した。フォトンカウント値では術後1日目に最高であった症例12例と，術後7日目に最高であった症例8例があったが，両群とも，他眼の前房内蛋白濃度と細胞数に有意の上昇は術後経過中観察されなかった。すなわち，交感性反応は見られなかった。

We tried to identify the consensual reaction in the anterior chamber in the fellow eye following posterior chamber lens implantation surgery. We quantitated the protein concentration and cells in the anterior chamber in the operated and the fellow eye in 20 cases during the postoperative period. A laser flare-cell meter was used in the quantitation.

The operated eyes responded with significantly increased protein concentration and cells in the anterior chamber on the first and 7th day after surgery. The contralateral eyes consistently failed to show changes in protein concentration or cells in the anterior chamber throughout the postoperative period. The findings were proof that no consensual reaction was elicited in the contralateral eye with our surgical approach in the present series.
