要約　目的：好酸球性多発性血管炎性肉芽腫症（EGPA）に大きな眼瞼結膜腫瘤を合併し，手術を施行した1例の報告。

症例：44歳，男性。他施設で40歳時にEGPAと診断され，ステロイド剤と免疫抑制薬の治療を継続中であった。喘息，血清好酸球増加，発熱，紫斑，鼻炎・中耳炎，心囊液，ステロイド性糖尿病の既往がある。他施設で両眼の眼瞼結膜乳頭増殖病変の切除術も受けていた。当院初診時，右上眼瞼瞼結膜に13mm×10mm×4mmの大きな結膜腫瘤を認めた。薬物治療では改善しないため，外科的治療を行った。病理組織診断で悪性の所見はなかった。手術後，13か月間再発していない。

結論：薬物療法では改善しないEGPAの眼瞼結膜腫瘤は，外科的治療が短期的に有効であった。EGPAは慢性の疾患であり，結膜病変の再発する可能性を念頭に置き，注意深い経過観察は必要であると考えた。

Abstract　Purpose：To report a case of eosinophilic granulomatosis with polyangiitis who showed a tumor in the tarsal conjunctiva.

Case：A 44-year-old male presented with a tumor in the upper tarsal conjunctiva in the right eye. He had been diagnosed with eosinophilic granulomatosis with polyangiitis 4 years before and had been treated with peroral corticosteroid and immunosuppressive agents. He had received resection of tumor in the upper eyelid in both eyes.

Findings and Clinical Course：The right eye showed a large tumor in the upper palpebral conjunctiva. The resected specimen showed no malignancy when examined pathologically. There has been no recurrence for 13 months after resection.

Conclusion：Resection was effective for a short period after resection of tumor in the tarsal conjunctiva in a case of eosinophilic granulomatosis with polyangiitis.
