Abstract Purpose:To report a case who received conventional reconstruction surgery for bilateral cataract and lens coloboma.

Case:A 55-year-old male presented with impaired visual acuity since a few years before.

Findings and Clinical Course:Visual acuity was 0.04 right and 0.1 left without correction. Both eyes showed marked horizontal astigmatism. Corrected visual acuity was 0.7 right and 1.2 left. Both eyes showed nuclear cataract and notch-like lens coloboma at 4 to 6 o'clock position. Lens zonules were absent in the area of lens coloboma. Both eyes received phacoemulsisfication and aspiration, and intraocular lens insertion. Visual acuity was 1.2 in either eye after surgery. There has been no problem for 6 months until present.

Conclusion:This case illustrates that conventional lens reconstruction surgery may be safe and effective for mild lens coloboma with no fragile lens zonule.
