壮年期に発症した傍中心窩網膜細動脈瘤の1例

Retinal macroaneurysm in the parafoveal area in a presenile adult

要約　背景：網膜細動脈瘤（retinal macroaneurysm）が傍中心窩部に生じることは稀である。

目的：網膜細動脈瘤が傍中心窩部に生じた壮年者の報告。

症例と経過：50歳の女性が右眼の傍中心暗点で受診した。矯正視力は右1.0，左1.2で，右眼の中心窩に接して0.3乳頭径大の内境界膜下出血があった。4週後にこの出血は0.5乳頭径大になり，インドシアニングリーン蛍光眼底造影で網膜細動脈瘤と推定される赤色点が中心小窩より512μm離れた部位にあった。視力は7週後に0.08，12週後に0.15，19週後に0.8になった。

結論：本症例は傍中心窩部に生じた網膜細動脈瘤の稀有な例と考えられた。

Abstract　Background: Retinal macroaneurysm is known to rarely occur in the parafoveal area.

Purpose: To report a case who showed retinal macroaneurysm in the parafoveal area.

Case: A 50-year-old woman presented with blurring near the point of fixation in the right eye.

Findings: Corrected visual acuity was 1.0 right and 1.2 left. The right eye showed a dot hemorrhage nasal to the fovea. Its size was 0.3 disc diameters（DD）and was located posterior to the internal limiting membrane. Its size was 0.5 DD four weeks later and 512 μm away from the foveola. It was identified as retinal macroaneurysm by IA. Visual acuity was 0.07 seven weeks later, 0.15 twelve weeks later, and 0.8 nineteen weeks later.

Conclusion: The present case was diagnosed as an unusual incidence of retinal macroaneurysm in the parafoveal area.
