過去5年間のサイトメガロウイルス網膜炎6症例の検討

Review of six cases of cytomegalovirus retinitis in the past five years

要約　目的：最近5年間に経験したサイトメガロウイルス（CMV）網膜炎の全身状態の報告。

対象と方法：2015年までの5年間にCMV網膜炎と診断された6例7眼を診療録の記述に基づいて検討した。

結果：男性4例5眼，女性2例2眼で，年齢は33〜69歳，平均57歳である。基礎疾患として，白血病などに対する骨髄移植後の移植片対宿主病3例，類天庖瘡1例，関節リウマチ1例，糖尿病1例があった。5例がステロイド薬と免疫抑制剤の投与中であり，3例が中心静脈栄養中，1例がカリニ肺炎を発症していた。糖尿病の1例では，血糖のコントロールは良好で，血液の免疫機能は正常であった。CMV網膜炎が再発した2例3眼では，基礎疾患が悪化していた。

結論：CMV網膜炎は免疫抑制状態で好発したが，免疫機能が正常でも発症することがある。

Abstract　Purpose: To report systemic features of cytomegalovirus（CMV）retinitis observed during the past 5 years.

Cases and Method: This retrospective study was made on 7 eyes of 6 cases who were diagnosed with CMV retinitis in the past 5 years.

Results: The present series comprised 5 eyes of 4 males and 4 eyes of 4 fvemales. The age ranged from 33 to 69 years and averaged 57 years. As underlying disease, 3 cases had graft versus host disease following bone marrow transplantation, one had penphigoid, one had rheumatoid arthritis, and one had diabetes mellitus. Five cases had been receiving corticosteroid and immunosuppressive drugs, 3 had been receiving intravenous hyperalimentation, and one had Pneumocystis carinii pneumonia. One case had diabetes mellitus with well-controlled blood sugar and normal immune condition in the blood. General conditions had exacerbated in 3 eyes of 2 cases at the time of recurrence of MCV retinitis.

Conclusion: CMV retinitis was frequently but not necessarily associated with immunosuppressive status.
