Type III Fucosidosisの三姉妹例—眼症状ならびに結膜生検所見

Ocular findings in type III fucosidosis

（1）低身長，gargoyle様顔貌，短頸，全身皮膚の被角血管腫（angiokcratomacorporis diffusum），精神運動発達遅延，神経症状，骨異常を呈し，末梢血白血球および，培養皮膚線維芽細胞中のα—L-fucosidase活性が著明に低下〜消失していたtype III fucosi—dosisの三姉妹例を検査する機会があり，種々の興味ある眼科的所見を得たので報告する。

（2）角膜には，実質深層に微細な点状白色混濁がび漫性に存在し，輪部から約1.5mmの透明帯が認められた。

（3）結膜・網膜血管はビーズ様，一部aneurysma様変化があり，視神経乳頭付近の後極部静脈の口径不同が強く，螢光眼底造影により，拡張血管に組織染が認められ，血管内皮細胞の障害が証明された。

（4）結膜生検を行い，結膜上皮細胞，線維芽細胞，血管内皮細胞，Schwann細胞，macrophageに多量の細胞内沈着物を認めた。

　組織化学的検索にて，多量の細胞内沈着物はfucoseを含む事を確認した。

　また，電顕にて上記細胞内に2種類のinclusion，すなわち微細なレース様構造物を含むclear inclusionと均一なdense inclusionを認めた。各inclusion内には，myeline様層状構造を示すvesicleも存在していた。

Three sisters with type III fucosidosis were evaluated as to the ocular manifestations. Biopsy specimens of the conjunctiva were studied by his-tochemistry and electron microscopy. Blood vessels in the bulbar conjunctiva showed prominent dilatation and tortuosity. A fine, diffuse opacity was seen in the deep stroma of central cornea. Retinal veins were dilated and tortuous with a beaded, sausage-like configuration. Fluorescein angiography showed staining of venous wall.

The presence of fucose within the cytoplasm of both the epithelial cells and the macrophages was observed by histochemistry.
