蓄膿症手術後一眼の眼窩内に発現した下垂体道腫瘍例

I．緒言

　副鼻腔蓄膿症手術後に眼瞼の腫張，眼痛，眼筋麻痺，眼窩膿瘍等が発生する事がある。これ等の多くは，適当の処置で間も無く回復するのが常であるが，稀れには重症の経過を取る。副鼻腔の手術は直視下で行われる事は少なく，又，眼窩骨壁が異常に菲薄で有つたり，或は欠如する場合さえもあり，又，其の他の畸形があるから起こる障碍であり，真に止むを得ぬ場合が多い。従つて，副鼻腔手術後，手術側の眼球突出症や眼瞼下垂，眼球運動障碍が起こつた場合は副鼻腔手術の異変に原因を求めるのが吾々の常識である。然し今回は私共は思いがけない症例に遭遇した。

　下垂体腫瘍の発生の場合は，殆んどが同時に，両側性の眼症状を現すのが常であり，一眼に眼球突出，瞼下垂，眼球運動障碍，甚しい視力低下を来して，他眼は余りにも変化が無いと云う場合は稀れである。然し異所的に発生した下垂体腫瘍であつた場合は例外であろう。私は今回発行された所安夫助教授の脳腫瘍を手にして，異所性（dysto—pisch）の下垂体腫瘍がある事を知り，始めて納得する事の出来た例症を経験した。私共は二つの珍らしい内容を持つた症例に遭遇したので，その経過を報告し大方の御批判を仰ぎ度いと思う。

A woman, aged 63, complained of vertigo and headache and was operated upon for right frontal sinus empyema. Since then she noticed exophthalmos and ptosis of only the right eye, her right eye sight was failing and the right eye was immovable, but the left was intact.

Exentratio orbitae and afterwards pathologic anatomy was carried out. Histologically the tumor shows an alveolate structure and consists of large and small squamous cells, but they are not keratohyaloid or keratose and their kernels ar reich in chromatin. Such a tumor is not derived from a paranasal sinus, but the canalis hypophyseos.
