まえがき

　本症例は，交通事故により顔部より頭部にかけて広範囲に受傷し，前頭部より眉毛部にかけて既に前後3回の手術をうけたが，瘢痕性外反症を胎したために吾々の眼科を訪れたものである。

　形成手術は，形態的な損傷を治癒せしめると同時に，機能的な回復を求めることが大切な目的である。眼部における形成手術は，眼に特有な機能回復をはかるために，時としては形態的な不充分さを忍ばねばならぬことさえある。例えば，正面位に於て，瞼裂が左右つりあつたとしても，瞼の閉鎖が出来なければ，兎眼症による眼障害が早晩失明の危険を招く可能性のあることを考慮しなければならない。このような意味で，例えば顔面神経麻痺の兎眼症に対しては，左右の瞼裂に形態的不均等を来す結果になつても，瞼板縫合により瞼裂縮小術をおこなうべきである。

The authors performed the plastic operation employing a free transplant of skin on a patient leaving cicatric ectropion and lagophthalmos after injury caused by traffic accident. This case was injured on the head extending to the face and in general surgery was op-erated on frontal area extending to eyebrow by means of transplantation. There was the direct adhesion between the eyebrow and the lid margin, and the latter was pulled up just under the former. When this adhesion was being dissected, the upper lid was filled with connective tissues and there were found neither orbicular nor levator muscles.
