小児眼手術に於ける全身麻酔

General anesthesia in operation upon eye of infants.

I．緒言

　従来眼科手術の施行に際しては，表面麻酔，浸潤麻酔，伝達麻酔等の眼局所麻酔により，その目的を達して来たのであるが，それは，あくまでも患者の手術に対する納得と協力を前提条件としてであつた。所で，未だ出生後日の浅い乳児は疼痛反応は反射運動として現われて，大脳皮質機能の発現ではないし，又この時期の乳児には，助手の用手的固定介助によつて充分手術目的を達する事が出来る。

　しかし，反射興奮性，基礎代謝の最大となる幼児期に於ては，智能の発達は感情の発達より遅れ，行動は凡て感情に支配されて，理解力，抑制力に乏しく，暗示も困難である。斯る幼児期の手術を如何に理想的に行うかは，反射興奮性を低下せしめ，感情の発現を抑制する全身麻酔処置によつて成立するのであるから，眼手術に於て全身麻酔が適用となるのは，満1歳より満8歳位迄の小児である。尚智能発育面より低能者が，感情面より神経質者が同様に全身麻酔の適用と認められる。

General anesthesia is necessary in the operation upon eye of children from 1 to 8 years old, of mental defectives, and of men nervous temperament who cannot keep quiet by dint of understanding, cooperation or suggestion when the fixation of patients by assistant is impossible.

Authors investigated various general anesthesia given on 27 children who came to or was accomodated in Ophthalmic Clinic of Iwate Medical College and obtained the following re-sults.
