緒言

　白内障の冷凍摘出は1961年Krwawicz¹）の発表以後，急速に改良普及したが，その手術時の輪部切開の大きさに関して，半周以上を可とする意見から1／4周でよいとするものまで，さまざまな見解が内外において述べられた^2〜6）。しかしそれらの見解は必ずしもdataに立脚して示されたものではなく，また小切開による全摘出への批判は硝子体脱出および破嚢の危険増大の指摘にあるので，私は実施した各種の大きさの切開による白内障全摘出の成績を纒め，それらの問題を中心に検討し一応の知見をえたので報告する。

Intracapsular cataract extraction was done for 172 eyes through various sizes of incision ranging from 90 to 180 degrees of the circum-ference of the corneal limbus. The visual acui-ty after the surgery for senile cataract was 0.83 in cases of large, or more correctly, 180-degree incision, and 0.84 in cases of small or under-170-degree incision. Post-surgery astigma-tism was also similar between the cases of the large and the small incision. Vitreous loss occurred in 11.4% of the large incision surgery and in 9.2% of the small one, some of which seemed to be induced by the lack of experience.
