網膜静脈枝血栓症に対する光凝固術

Photocoagulation in Branch Thrombosis of Retinal Vein

緒言

　網膜静脈の一枝に血栓が生じると，その支配領域に神経線維の方向に一致した出血がみられる。なかでも最も頻度の高いものは，上耳側静脈，ついで下耳側静脈である。これら一枝だけの閉塞では，網膜中心静脈血栓症のごとく，突然の視力低下を訴えることはないが，それらがいずれも黄斑部の上方もしくは下方に位置するため，徐々に黄斑部に病変が及び，出血斑，白斑，および黄斑浮腫をきたし，視力低下につながる。また網膜静脈枝血栓症は，自然にあるいは薬物療法によつて，きわめて緩慢ではあるが治癒することが知られている。しかし，その経過の長いこと，視力の予後が保障されないこと等不満足な面が多々ある。著者らはこの度，網膜静脈血栓症の症例に光凝固術を試み著効をみたので報告する。

Nine patients with branch thrombosis of re-tinal vein were treated by photocoagulation. The lesion in eight of them occupied the supe-rior temporol quadrant of the retina. Photocoa-gulation resulted in enlargement of the avascular area and separation between the main superior temporal vein and its collateral anastomosing branches, causing a recovery of the retinal cir-culation.

Vision improved in all patients and no com-plications were noted after photocoagulation.

These results indicate that photocoagulation is the most suitable treatment for this disease.
