先天性白内障に関する研究—その2先天性風疹症候群

I．緒言

　先天性白内障は，先天異常の成因の解明に関する研究には，またとない疾患の一つといえる。一般に先天異常の成因は，1）遺伝，2）環境，3）特定の遺伝子を指摘できない遺伝要因と，不明の環境要因の合力による3つに大別される。Neel （1958）は，16,000人以上の小児について異常の成因を追及し，遺伝によるものは20％，染色体異常によるもの10％，ウイルスによるもの10％，残余の60％は、特定の成因が明らかでなく，遺伝と環境因子の合力によるものと推定している。先天性白内障の成因については，Verrey （1957）は，89例について調査し，そのうち23例（25％）しか成因を明らかにし得なかつたと報告している。23例の内訳は，10例が遺伝性，3例が風疹ウイルス，8例は慢性あるいは急性疾患によるとしている。

　先天異常の環境因子として，現在までに明らかにされているものは，放射線，化学物質（サリドマイド，アミノプテリン），ウイルス（風疹，巨細胞封入体症），トキソプラスマ，酸素欠乏などがある。この中で，眼奇形に関係するのが明らかに証明されているものに，風疹，トキソプラスマ，サリドマイドがある。このうち，先天性風疹症候群が最もよく研究されている。

Two cases of congenital rubella syndrome, confirmed virologically, were reported.

Case 1 : a 1,400 gm. female baby, borne at 34 week's gestation of a mother who had had ru-bella during the first two months of pregnancy. Thrombocytopenic purpura, congenital heart disease, hepatosplenomegaly and bulging ante-rior fontanelle were noted on physical examina-tion. Ocular exaimnation revealed the central nuclear cataract of the right eye and the rubella retinopathy of the left eye. Rubella virus was recovered from the throat, and urine on 50th day of life. One eye was obtained postmortem for histopathologic study.
