外来における1歳未満児のトラコーマ追求，殊にその初発について

Trachoma of babies in our clinic and its initial symptoms

　Journal of Japanese Association Against Trachomaトラコーマは急性に発病するものであるか，それとも気づかれなく徐々に発病するのであるかの問題は古くより論議され，ことに第二次大戦後わが国では急性発病が強調せられ，それに対して賛否が闘われてきたのは周知の事である。人体接種に由る実験トラコーマ（以下トラコーマはトと略す）の発病は急性であるとされているが，日常我々が外来で接する自然トでは，実際に個々の発病形式，殊に初発状態を知り得る事は色々の条件の加わるために極めて難しい。従つて容易にトの急性発病説を納得しきれないのが真情であろうと思う。

　さてトの初発状態を知るには，出生より結膜を長期に亘り観察して行くのが最良の方法であるが，この様な研究をした人は意外に極めて少ない。宮下等（昭17）と松永等（昭30）の家庭を継続検診して得た秀れた業績があるにすぎない様で，外来においての研究は寡聞にして知らない。私は外来でも，出生直後からの一様な観察は不可能にしても，それに近い方法を採つてトに成る乳幼児を探せばトの初発形式はどの様なものであるか理解し得るのではないかと考え，以下の様な試みを企てた。

A report is given over the conjunctival findings of 130 babies rangeing from 7 days to 11 months of age with no prior treatment in the author's outpatients clinic. Trachomawas detected in 3 cases (2.3%) among the cases examined.

Subsequent follow-up studies for 21/2 to 5 years revealed the development of trachoma in further 10 cases (7.9%).

From the above experience a view is pre-sented that the trachoma in babies has its onset within 6 moths after birth with initial symptoms of acute to subacute catarrhal or follicular conjunctivitis in the majority of cases.
