要約　目的：眼内レンズ（IOL）亜脱臼を合併した落屑緑内障2眼に対するIOL摘出と線維柱帯切開同時手術の手術成績の報告。症例：2例ともに90歳代の高齢者であった。白内障手術と眼内レンズ挿入術を過去に受けていた。硝子体腔へのIOL亜脱臼に合併して眼圧上昇をきたしたため，IOL摘出および線維柱帯切開術を施行した。1例はIOL縫着術も同時に施行した。結果：2例ともに術中に重篤な合併症はなかった。1例は術後に一過性の前房出血と硝子体出血を認めた。術後眼圧は良好にコントロールされ，術前の視機能を維持している。結論：IOL亜脱臼を合併した落屑緑内障2眼に対し，本術式は有効であった。

Abstract.　Purpose：To report 2 cases that received simultaneous trabeculotomy and extraction of intraocular lens（IOL）for exfoliation glaucoma and IOL subluxation. Cases：The 2 cases were aged 90 and 93 years respectively. They showed subluxated IOL and corneal edema in one eye each. Intraocular pressure（IOP）in the affected eye was 34 mmHg and 36 mmHg each. Both cases were treated by simultaneous trabeculotomy and IOL extraction. One case received additional scleral fixation of IOL. Both cases showed uneventful intra-and postoperative course with normalized IOP. Conclusion：The present cases show that simultaneous trabeculotomy and extraction of intraocular lens may be safe and effective for IOL subluxation and exfoliation glaucoma.
