要約　目的：特発性眼窩炎症で眼球が後方から圧迫され，瞳孔ブロックが生じ，レーザー虹彩切開術が奏効した症例の報告。症例：60歳男性が12日前からの左眼充血と腫脹で受診した。所見と経過：矯正視力は右1.2，左0.6で，左眼に－3.5Dの近視があった。左眼の前房が浅く，眼圧は右16mmHg，左22mmHgであった。磁気共鳴画像検査（MRI）で左眼球の外後方に陰影があり，眼窩の炎症または腫瘍が疑われた。レーザー虹彩切開術で前房深度は正常化し，左右の眼圧がほぼ等しくなった。7日後に他施設で特発性眼窩炎症と診断され，ステロイドパルス療法で寛解し，左眼視力は－1.5Dの矯正で1.5に改善した。結論：本症例では，特発性眼窩炎症で眼球が後方から圧迫され，瞳孔ブロックが生じ，レーザー虹彩切開術が奏効したと解釈される。

Abstract.　Purpose：To report a case of idiopathic orbital inflammation with suspected pupillary block by pressure to the eyeglobe. Case：A 60-year-old male presented with swelling and hyperemia in the left eye since 12 days before. Findings and Clinical Course：Visual acuity was 1.2 right and 0.6 left when corrected by －3.5 diopters. Intraocular pressure（IOP）was 16 mmHg right and 22 mmHg left. The left eye showed shallow anterior chamber. Magnetic resonance imaging（MRI）showed a soft mass posterior temporal to the eyeglobe suggesting orbital tumor or inflammation. Laser iridotomy resulted in deepening of anterior chamber with decrease in IOP in the left eye. The patient was diagnosed with idiopathic orbital inflammation in another institution 7 days later. Pulsed corticosteroid therapy induced clinical cure with left visual acuity of 1.5 when corrected by －1.5 diopters. Conclusion：Orbital inflammation may have induced pupillary block by pressure to the eyeglobe in the present case.
