Abstract. Purpose:To report a case of endogenous bacterial endophthalmitis presumably secondary to pyogenic spondylitis. Case:A 68-year-old male presented with impaired vision in the right eye since 11 days before. He had had fever and lumbago since 20 days before and been found to have elevated CRP level. Findings:Corrected visual acuity was hand motion right and 0.9 left. The right fundus could not be observed due to dense vitreous opacity. Vitreous surgery led to the diagnosis of endogenous bacterial endophthalmitis. Causative site of infection could not be identified. Gallium scintigraphy and magnetic resonance imaging(MRI)led to detection of pyogenic spondylitis in the waist 17 days after surgery. Staphylococcus aureus was isolated from the blood and waist abscess. Conclusion:This case illustrated that pyogenic spondylitis may cause endophthalmitis. Its diagnosis was facilitated by gallium scintigraphy and MRI.
