アルガトロバン点眼がフィブリン抑制に有効であった穿孔性眼外傷の2例

Instillation of argatroban inhibited fibrinous exudation in two cases of penetrating ocular injury

要約　穿孔性眼外傷後に瞳孔領にフィブリンが出現し，眼底管理が困難になった2症例に，フィブリンを消退させ眼底の視認性を得る目的で，アルガトロバン0.05％の点眼を行った。症例1は受傷翌日から前房にフィブリンが出現し，アルガトロバン点眼を1日6回点眼した。投与3日後からフィブリンは減少し，6日後に消失した。症例2は硝子体内に異物を認め，硝子体手術を行った。術翌日からフィブリンが出現し，アルガトロバンを1日4回点眼した。点眼開始から6日後にフィブリンは減少し，10日後に消失した。抗凝固作用による出血や点眼液の眼刺激症状などの副作用はなかった。感染などでステロイドの使用が困難な症例ではアルガトロバン点眼は有用な治療手段である。

Abstract.　We treated two cases of penetrating ocular injury by topical instillation of 0.05％argatroban ophthalmic solution. The purpose was to prevent fibrinous exudation in the anterior chamber and to regain visibility of the fundus. The first patient developed fibrinous exudation in the anterior chamber on day 2 of injury. Instillation of argatroban 6 times daily resulted in decrease of exudation 3 days later and in resolution 6 days later. The second patient had foreign body in the vitreous. Fibrinous exudation appeared on day 2 of vitreous surgery. Instillation of argatroban 4 times daily resulted in decrease in fibrinous exudation 6 days later and in its resolution another 4 days later. These cases show that argatroban may be useful to inhibit fibrinous exudation following penetrating eye injury,particularly when corticosteroid is contraindicated because of infection.
