流行性耳下腺炎に併発した角膜内皮炎の1例

A case of mumps with corneal endotheliitis

　24歳女性が前日からの左眼視力低下で受診した。有痛性腫脹が7日前に右耳介，6日前に左耳介に生じ，39.7℃に発熱した。矯正視力は右1.2，左0.01で，左眼眼圧が34mmHgであった。左眼に強い毛様充血と角膜実質の浮腫があった。流行性耳下腺炎に併発した角膜内皮炎と続発緑内障を疑い，副腎皮質ステロイドの点眼を開始した。初診の10日前に長男が流行性耳下腺炎に罹患していること，定型的な全身症状，ペア血清によるムンプスウイルス抗体値の上昇から，流行性耳下腺炎の診断が確定した。角膜内皮炎は40日後に治癒した。左眼の角膜内皮細胞密度は1,442mm²で，右眼の約50％に減少していた。流行性耳下腺炎に併発した角膜内皮炎は早期に治癒するが，角膜内皮細胞の減少があるので注意が必要である。

A 24-year-old female presented with decreased vision in her left eye since the previous day. She had had high fever and painful swelling in both earlobes for about one week. Her corrected visual acuity was 1.2 right and 0.01 left. The left eye had intraocular pressure of 34mmHg with severe ciliary injection and corneal stromal edema. She was given topical corticosteroid for keratitis and secondary glaucoma. Diagnosis of mumps was established because of epidemic parotitis in her eldest son,typical systemic manifestations of mumps,and elevated titer of serum hemagglutination inhibition test against mumps antibody. Her corneal endotheliitis healed 40 days later. The corneal endothelial cell population in the left eye was 1,442mm²and was reduced to about 50％of that in her right eye. This case illustrates that mumps keratitis may improve soon but that it may cause marked reduction in corneal endothelial cells.
