北里大学における片眼性眼球突出の検討

Review of unilateral exophthalmos at Kitasato University Hospital

要約　目的：片眼性眼球突出の自験例の報告。対象と方法：過去25か月間に片眼性眼球突出と診断した17例を対象とし，診療録の記述をもとに検索した。結果：男性12例，女性5例であり，年齢は21～68歳（平均54歳）であった。炎症性偽腫瘍7例（41％），甲状腺眼症3例（17％），眼窩蜂巣炎2例（12％）などがあった。主な臨床症状は，結膜充血88％，眼球運動障害76％，結膜浮腫47％であった。治療として，視力または眼球運動障害が強い例にはステロイドパルス療法，軽症例には副腎皮質ステロイド薬内服，眼位または眼球運動障害に対しては手術を行った。結論：良性疾患が片眼性眼球突出の主要原因であった。

Abstract.　Purpose：To report the incidence and clinical features of unilateral exophthalmos in our institution. Objects and Method：This retrospective study was made on 17 cases of unilateral exophthalmos diagnosed during the foregoing 25 months. Clinical records served as chief material of analysis. Findings：The series comprised 12 males and 5 females. The ages ranged from 21 to 68 years,average 54 years. The causative lesions included inflammatory pseudotumor 7 cases（41％）,thyroid ophthalmopathy 3 cases（17％）and orbital cellulitis 2 cases（12％）. Chief clinical manifestations were conjunctival hyperemia 88％,impaired ocular motility 76％,and chemosis 47％. Pulsed corticosteroid therapy was performed for cases with severe impairment of visual acuity or ocular motility. Less severe cases received peroral corticosteroid. Impaired ocular motility or eye position was surgically corrected. Conclusion：Benign diseases were main causes of unilalteral exophthalmos in the present review.
