　眼瞼脂腺癌4例を経験した。38歳，62歳，71歳の男性と，79歳女性であり，いずれも繰り返す霰粒腫として近医で2～5年間加療されていた。4例すべてに眼瞼全層の広範囲切除を行い，迅速病理診断で断端に腫瘍細胞がないことを確認したのち切除量に応じて眼瞼を再建した。平均4年間の術後観察で再発または転移はなく，眼瞼の開閉機能と整容面はともに良好である。再発する霰粒腫では，年齢にかかわらず脂腺癌を疑い病理学的に検索をする必要がある。

　We observed four cases of sebaceous gland carcinoma of the eyelid. The series comprised one female aged 79 years and 3males aged 38,62 and 71 years respectively. All the cases had been treated as recurrent chalazion for 2 to 5 years before visiting us. Each patient was treated by extensive full-thickness resection of the eyelid. The excised specimen was examined histopathologically on the spot to confirm the absence of tumor at its edge. The eyelid was reconstructed to replace the tarsoconjunctival defect. There has been no recurrence or metastasis during the follow-up averaging 4 years. The patients have been doing well regarding the eyelid function and appearance. We advocate histopathological examinations in cases of recurrent chalazion regardless of the age of the patient.
